About Us |
MISSIONTo promote the optimal level of independence for elderly and disabled adults, maintain their present level of functioning as long as possible, promote support and respite for families and other care-givers, foster socialization and serve as an integral part of the community service network.
REFERRALSOSIP Adult Day Services welcomes inquiries from individuals interested in attending our program. We also receive referrals for potential clients from family members, doctors and social workers. OSIP Adult Day Services has a reciprocal referral relationship with the Visiting Nurse Association (VNA) and the Central Vermont Council on Aging (CVCOA) case managers to ensure that area seniors get the services they need. The Vermont Department of Disabilities, Aging and Independent Living (DAIL) is regionally staffed with Long-Term Clinical Care Coordinators to assist in identifying and referring persons needing adult day services. |
