The majority of our Participants are elderly, as an aging population is the trend nationally, regionally, and locally. According to the 2000 census the residents of Orange County are significantly older than Vermont’s median age, which was older than the U.S. median age. The older a person is the higher the probability that he or she will have disabilities. In 1990, two percent of those over 65 were living alone. Living alone increases the probability that people do not receive informal long-term care, as is commonly provided by spouses. Based this information, Adult Day services will continue to be of particular interest because it represents an entirely different setting for care from the home or institution. Our goal is to serve people who are handicapped, impaired or frail older adults living within a 20-mile radius of Newbury Village. Adult Day services provide supportive services for individuals: · Living in isolation · Recently discharged from hospital or nursing home · Recovering from a major illness · Adjusting to loss of a family member · Adjusting to loss of function, or · Living with family and in need of time on their own. These are people living with confining health issues such as post-stroke symptoms, heart disease, and Alzheimer’s disease. The younger adults are either cognitively or physically challenged.
ELIGIBILITY GUIDELINES
Participants must be physically or mentally frail adults who are in need of daytime services in order to maintain or increase their level of independence. Admission to the program will be made on the basis of need, appropriateness of the program to the individual’s living situation and availability of space. The participant must be: · physically impaired to the extent that such impairments interferes significantly with their capacity for self-care · mobile with whatever mechanical aids needed, such as wheelchairs or walkers · exhibiting appropriate non-threatening behavior · free from infectious disease · free from active abuse of drugs and/or alcohol · continent or maintained with personal care devices · a willing participant in the Adult Day services program No individual shall be excluded from participation in or be denied the benefits of the Oxbow Senior Independence Program Adult Day Services by reason of age, disability, race, gender, sexual orientation, religion or national origin. Under the Americans With Disabilities Act of 1990 (ADA), any qualified individual with disabilities is eligible to participate in or benefit from any service, program or activity provided by OSIP Adult Day Services. The Executive Director or designee shall visit potential participants in their homes to assess individual needs, interests and eligibility. Applicants will be accepted on a one-month trial basis, after which an evaluation will be done to determine suitability for ongoing participation. HEALTH CONDITIONS
